http headers

this patch allows one to set http headers via the view.
for usecases, the headers may be specified in the rc, e.g.:

<http-headers>
  <header name="Cache-Control" value="no-cache, no-store"/>
  <header name="Expires" value="0"/>
  <header name="Pragma" value="no-cache"/>
</http-headers>
